Goal: Promote health equity for underserved populations by expanding access to services, promoting community-based prevention, and advancing health policy reform.

Objectives:

  1. Support policy and advocacy efforts to implement health reform so that underserved populations have access to culturally competent, quality, and affordable healthcare.
  2. Invest in community clinics and other healthcare safety net providers to assure access to culturally competent, quality, and affordable healthcare for underserved populations.
  3. Invest in community-based organizations, community leaders, and public health departments to plan and implement community prevention activities to reduce chronic disease rates and address health inequities.

American Heart Association of San Francisco To reduce risk and incidence of heart disease among African Americans in the Bay Area through faith-based health outreach, health screenings and behavioral change. $50,000 San Francisco
Asian Health Services To assure that underserved immigrant and refugee groups have a voice in health care reform implementation through AHS’ innovative patient leadership council program. $40,000 Oakland
Bay Area Black United Fund To support the implementation of a new organizational transition plan which includes launching a new mission and branding platform, and an online donor development initiative for BABUF and its family of health-related member agencies. $100,000 Oakland
Bay Area Regional Health Inequities Initiative To enable a strategic, regional collaboration of senior leadership and staff from eleven local health departments in the Bay Area to continue to work with communities and other local, state, and national partners to address the root causes of health inequities. $40,000 Oakland
BRIDGE Housing Corporation To fund the Healthy Generations Project, a community based health program to reduce health disparities in Potrero public housing by focusing on preventative developmental health measures in the areas of toxins, nutrition, emotional security, and cognitive enrichment. $80,000 San Francisco
California Budget Project To support analysis of state budget and budget-related policy issues, with a particular focus on the implementation of health reform, and related outreach and communications efforts. $35,000 Sacramento
California Pan-Ethnic Health Network To advocate for public policies to improve the health of communities of color, mobilize constituents through community convenings and the Having Our Say coalition, and develop and disseminate policy briefs. $40,000 Oakland
California School Health Centers Assn. To build on opportunities within health care reform at the state and federal levels to secure a sustainable role for school-based health centers in health care delivery and enrollment systems. $65,000 Oakland
Center for Youth Wellness To provide case management and educational advocacy for low income, urban children and youth affected by adverse childhood experiences, to reduce health disparities. $80,000 San Francisco
ChangeLab Solutions To develop the Healthy Places Community Assessment (HPCA), a step-by-step evaluative framework that will engage local stakeholders in identifying strategic opportunities for revising, amending, and creating healthy and sustainable programs, policies, and practices at the local level. $35,000 Oakland
Chinese Progressive Association To work within the Chinese immigrant community, and with other underserved communities, to build their capacity to address disproportionate environmental health risks and improve access to healthcare. $20,000 San Francisco

Hayward Day Labor Center To serve as an entry point to health care services for the hard to reach low-income day labor worker population in southern Alameda County and strengthen and improve health care admissions coordination among various community based and hospital providers. $40,000 Hayward
Health Access Foundation To expand and improve access to healthcare for uninsured and underinsured residents of the Bay Area through education and research; to prevent harmful budget cuts to public health; and to inform, implement, and improve health reforms at the federal, state, and local level. $40,000 Sacramento
Health Access Foundation To support the work of the LGBT Health and Human Services Network to implement health reform and state-level health regulations in ways that insure all LGBT people and their families, particularly those in need of safety-net services, have access to culturally inclusive health care. $40,000 Sacramento

Institute for Food and Development Policy To support the ongoing operation of the Oakland Food Policy Council (OFPC) to transform neighborhood food systems into engines for local economic development and improve access to healthy eating choices for underserved communities. $25,000 Oakland
La Clinica de la Raza, Inc. To build capacity and enhance infrastructure of School Based Health Centers to provide accessible no cost medical and health education services to low income underserved students in Oakland and San Lorenzo. $26,540 Oakland

San Francisco Department of Public Health To evaluate the SF Hep B Free campaign to determine which outreach strategies had the most impact on behavior change among 1) clinicians to provide hepatitis B testing, vaccination and appropriate follow-up testing for Asian Pacific Islanders (APIs) and 2) APIs to seek out and accept these services. $20,000 San Francisco
San Mateo County Union Communty Alliance To support SMCUCA’s outreach, community organizing, and advocacy in its Community Coordinating Project for Access to Health Care and to build SMCUCA’s capacity to plan and implement primary community prevention work with a focus on the root causes of health disparities. $80,000 Foster City
St. James Infirmary To provide free primary care, reproductive, and transgender health services for uninsured sex workers. $20,000 San Francisco
The Insight Prison Project To support rehabilitation programs for incarcerated individuals in prisons and jails that positively affect rates of institutional violence, successful and safe reentry into Bay Area communities, and individual health and well-being. $30,000 San Rafael
Transgender Law Center To increase access to medically and culturally competent health care in the Bay Area through Project HEALTH (Harnessing Education, Advocacy, and Leadership for Transgender Health). $25,000 San Francisco
